Community Services of the Guilford County Department of Public Health encompasses programs that are community based, meaning services are rendered to residents at places other than the health department. Some of these services are referred to as Home Visiting Services. Please read on for more information on some of the different types of Home Visiting Services we provide.
Family Connects Guilford
Family Connects Guilford is an evidence-based Nurse Home Visiting program that helps parents better understand how to care for their baby, increase their confidence, provide breastfeeding support, reduce emergency room visits, and reduce the incidence of child abuse and neglect. During a home visit, a Family Connects nurse will address a family’s questions and concerns, offer emotional support, check vital signs/assess the overall health of baby and birthing person, provide education, identify back-to-work and home support and make referrals as needed. Care Management for High-Risk Pregnancy (CMHRP)
The Care Management for High-Risk Pregnancy Program is a free care management service available to pregnant women enrolled in North Carolina Medicaid and to a limited number of uninsured pregnant women with chronic conditions and/or other needs.
Services are provided by trained registered nurses and social workers in the OB practice, home, or other settings. Care managers provide education, encouragement and support based on each individuals need throughout pregnancy and up to 60 days after giving birth.

Community Alternatives Program (CAP/C) for Disabled Children and Disabled and Elderly Adults (CAP/DA)
The Community Alternatives Program (CAP) for Disabled Adults (/DA) and Children (/C) is state-wide program that assists children and adults with Medicaid remain at home or in the community instead of being institutionalized. Services are provided by trained nurses and social workers. Case management responsibilities include assessments, care planning, regular monitoring, referral, and linkage to other services and supports to address identified needs.
Contacts will vary between in-person, telephone, e-mail, and video-based communication. CAP RNs also help monitor Guilford County Adult Day Health Day Care Centers and occasionally assist with other community health projects.
Care Management for At Risk Children (CMARC)
Care Management for at Risk Children (CMARC) specializes in early childhood comprehensive care management for families with children ages 0- 5 with (neonatal/chronic health conditions; social determinants of health: developmental delays or exposure to toxic stress/ trauma). Services are provided by specially trained social workers and nurses in the home or various settings. Care managers assist families with developing an individualized plan of care; coordination of community resources; comprehensive needs assessments and basic developmental screenings are also provided.
Child Fatality Prevention Team (CFPT)/ Community Child Protection Team (CCPT)
The Child Fatality Prevention Team (CFPT) was established to review child fatalities and to continually search for ways to prevent these fatalities from occurring. The Community Child Protection Team (CCPT) is a subset of the CFPT and its purpose is to review child deaths attributed to abuse and/or neglect. Both teams function as one to keep all children safe and advocate for services each family needs to succeed.
