The School Health Program supports Guilford County Schools through case management, health education and consultation in order to remove health related barriers to learning.

School nurses promote health, safety and educational success of school age children in Guilford County by serving students, families and school staff through partnerships, evidence based practice and professional standards of care.

Students are entering school with many more medical problems than in the past. For example, we now have children in classrooms who receive daily tube feedings. School staff are assisting with medical procedures such as catheterizations, learning to administer insulin to diabetic children and other emergency injections. School nurses provide the medical expertise, the training of teachers and staff, and the monitoring of students with routine and special needs. Guilford County Health and Human Services school nurses collaborate with other human services programs that address substance abuse, mental and physical illness, nutrition, dental and vision needs, obesity and preventive health care measures.

Health and education leaders recognize that there is a direct connection between health and learning. A healthy student learns better and is better able to meet their academic potential.

The completed form should be returned to your child’s School Health Nurse or school cafeteria manager.

Medication Authorization Form

If your child needs to take medication while at school, a medication authorization form will need to be completed and submitted to your child’s school.

Health Assessment Form

Every child that is new to North Carolina Public Schools, in grades K-12, is required to have a North Carolina Health Assessment Transmittal Form completed by a health care provider and submitted to the school within 30 calendar days of a child’s first day of attendance beginning with school year 2016-2017.

Asthma Education for Parents and Children

Asthma Tools for Parents

The American Lung Association has a number of resources to fit your needs of asthma education for parents and school personnel: Asthma Basics is a 50-minute online learning tool that teaches participants to identify and manage asthma symptoms, understand an Asthma Action Plan, and respond to a breathing emergency. Lungtropolis Parents is a fun tool that provides information and resources to caregivers of children with asthma. Lungtropolis Kids is the version for children.

Medication Demonstration Videos are an excellent way for parents to see if their child is properly using his/her medications. Downloadable direction sheets for eight medication devices are available.

What is Asthma? is a two minute online animation that explains what asthma is and how it affects people who suffer from it. Access it by clicking on the What is Asthma icon on the right side of the ALA Asthma webpage.

Lung HelpLine is staffed by experienced RNs and respiratory therapists who can answer parents’ questions on their child’s asthma symptoms, medications and more.

Asthma 101 is a one to two hour basic asthma education seminar for adults developed by the American Lung Association. This seminar is designed for use in a variety of settings including schools, child care, workplace and community meetings.
