Make Sure Your Student Is Ready for School - Guilford County Public Health Offers Saturday 7th Grade Immunization Clinics (Health Information)

The Guilford County Department of Health and Human Services (GCDHHS), Division of Public Health has scheduled special Back-to-School vaccination clinics for rising 7th graders, Saturday September 22, 2018 from 9:00 a.m. to 11:00 a.m. at 1100 E. Wendover Avenue in Greensboro and 501 E. Green Drive in High Point. An appointment is recommended and can be made by calling 336-641-3245 but walk-ins will be accepted.  This number will accommodate both English and Spanish speakers.  Appointments are also available 8:00 a.m. to 5:00 p.m. Monday through Friday (except holidays) in the Division of Public Health’s general immunization clinics.  Please bring your child’s insurance card (including Medicaid) and your child’s vaccine record to the appointment.  Fees may apply; please call 336-641-3245 for fee and insurance questions prior to your child’s appointment.

 Parents are required to provide proof of up-to-date vaccinations to the school within 30 days of the start of school.  A physical exam is also now required for any student new to the North Carolina public schools regardless of the grade level.  The required Health Assessment Form must be completed by your child’s health care provider.
